We ask that you provide us 24 hours notice prior to canceling or rescheduling your appointment for any reason, in order to avoid a $35 service charge.  
You will not be seen if you are 10 minutes late for any appointment, for any reason. Your appointment will be considered a no show, and you will be charged the $35 service charge.

Please arrive 10- 15 minutes earlier than your scheduled time to account for any traffic issues, filling out necessary paperwork and to process your payment.

I understand that any children brought to an appointment at Baby Envision must be supervised by another adult while I have my ultrasound. At no time are children of any age to be left unattended or unsupervised. If staff is unable to perform the ultrasound due to children being present, you will be charged $35 and your appointment will be rescheduled.*
